*Organization must serve children in need in San Luis Obispo County.

 

Please include the following:

1.      Describe your specific needs for these funds including number of children served.

2.      What is your non-profit status?  (501(c)(3) or 501(c)(2) or ?) 

       Please include a copy of your 501(c)(3) if applicable.

3.      Do you receive government funding?

4.      How many paid full-time and part-time staff do you employ?

5.      If you received funds from us last year, how were they spent?

6.      Provide your current year’s budget and a performance budget from last year.  If this request is part of a larger program, please provide a budget specific to this request.

7.      Give a brief history of your organization and a list of the members of your Board of Directors.

8.      How will our donation be recognized?

 

  • Grant applications must be received (not postmarked) by September 30th.
  • E-mail applications will not be accepted.
  • Please provide seven copies of your application.
  • Grants will be awarded in December.
